These rank flags were first published in Album
des Pavillons (2000). For all of the Vice Admiral, Rear Admiral, and
Commodore flags and pennant, the lion is red - I saw the flag two months ago in
Colombo. Album
des Pavillons (2000) earlier showed it as golden, but that is an error.

According to Album des Pavillons (2000), the
ratio is given as ~2:3.

A white flag with a red anchor ensigned with a red lion passant holding
the sword and in base three red six-pointed stars, two and one. The red used
here is neither the brown-red of the national flag, nor the "normal" red, but
kind of a carmine. Just as the red in national flag is often replaced with
normal red in practice, I guess it may be case here, too.
